  • In compliance with the state of Florida section 849.094(3), F.S., for your chance to win two tickets to the Blackout Circuit Tour at Calvary Assembly Church in Winter Park, Florida on Saturday, May 31 all you have to do is completely fill out this entry form.

    This contest is sponsored by Z88.3 and Hot 95.9 which is located at 1065 Rainer Drive, Altamonte Springs, FL 32714. You must be a Florida resident, 18 years or older and have a photo ID to enter and win. Entries will be accepted Thursday, May 15 through Wednesday, May 28 at 4:00 p.m. One entry per household will be accepted. All entries will be placed into an Excel spreadsheet and any duplicate entries will be deleted.

    The spreadsheet is set to number each valid entry as one record. The Hot 95.9 Promotions Department will use www.Random.org to randomly select 5 random numbers in the range of the number of entries. The winners are the corresponding record numbers in the spreadsheet.

    Winners will be notified by Thursday evening May 29. Each winner will receive two tickets, and the prize is valued at $30. Tickets must be picked up at Calvary Assembly on the day of the show. No purchase necessary. Transportation and parking not included. Odds of winning depend on the number of completed entries received.
